WI CCS Employee Responsibilities:

Success4Life staff providing services under the Wisconsin Comprehensive Community Services (CCS) program are responsible for supporting individuals in developing skills that promote recovery, independence, and overall well-being.

  • Provide CCS services including Individual Skill Development & Enhancement, Employment-Related Skill Training, and Wellness Management & Recovery

  • Work one-on-one with youth and adults to support recovery and personal goals identified in the CCS service plan

  • Assist individuals in developing daily living skills, coping strategies, and community integration

  • Support employment readiness and workplace skill development when applicable

  • Maintain accurate and timely documentation in accordance with CCS program requirements

  • Communicate regularly with the CCS service facilitator and treatment team

  • Follow all county CCS policies, procedures, and Medicaid documentation standards

  • Maintain professional boundaries and confidentiality at all times

  • Participate in required trainings and supervision

  • Represent Success4Life professionally when working with clients, families, and community partners

Employment Requirements:

Applicants interested in joining the Success4Life team should meet the following requirements:

  • Bachelor’s degree required (education, social work, psychology, or related field preferred)

  • Experience working with youth, young adults, or individuals with diverse needs

  • Experience working with at-risk populations preferred

  • Ability to provide mentoring, life skills coaching, or academic support

  • Strong communication and interpersonal skills

  • Ability to work independently and manage a flexible schedule

  • Reliable transportation and ability to meet clients in community settings

  • Valid driver’s license

  • Ability to pass a background check

  • Commitment to maintaining professional boundaries and confidentiality

  • Willingness to complete training and onboarding requirements

Colorado Mentoring & Life Skills Employee Responsibilities:

Success4Life staff in Colorado provide individualized mentoring, tutoring, and life skills coaching to support youth and young adults in building confidence, independence, and success in daily life.

  • Provide one-on-one mentoring and life skills coaching to assigned participants

  • Assist individuals in developing organization, communication, decision-making, and problem-solving skills

  • Support academic progress, goal setting, and personal development

  • Help participants build independence and healthy routines

  • Meet with participants in appropriate community settings such as schools, homes, or public locations

  • Maintain accurate records of services and participant progress

  • Communicate regularly with Success4Life administration regarding client needs and progress

  • Maintain professional boundaries and confidentiality

  • Follow all Success4Life policies and program guidelines

  • Represent Success4Life in a professional and positive manner within the community
